A beloved figure in Grundy Center, Philip “Coach” Lebo built a decades-long legacy that extended far beyond the field. Known simply as “Coach” to generations of students, he dedicated his life to teaching, mentoring, and shaping young lives through sports and the classroom. While championships and honors marked his career, it’s the lasting relationships and community impact that define him—proof that his greatest achievement was never the scoreboard, but the lives he influenced.

Creekside Received Third Consecutive Deficiency-Free Rating from State Survey
Creekside Living has once again received a Deficiency-Free Rating from the Iowa Department of Inspections and Appeals during our annual survey. This marks our third consecutive deficiency-free result — an achievement earned by only a small percentage of senior living communities across the state.
New assisted living community opens at Creekside Living in Grundy Center
“We saw a need for this among people who don’t require a nursing home but could still use some help to maintain their independence,” explains administrator Scott Kramer. “Assisted living is really a social model that can accommodate individual needs and make it possible for everyone to enjoy life to the fullest.”
